Reliq’s powerful iUGO Care software platform provides a virtual care system that supports care coordination and community-based healthcare. iUGO Care allows patients with complex care needs to receive high-quality healthcare across the continuum of community settings.
Our platform provides real-time access to remote patient monitoring data, allowing for timely interventions by the care team to prevent costly and disruptive hospital readmissions and ER visits. Working with clinicians, patients, and partners, iUGO Care is improving health outcomes, enhancing the quality of life for patients and families, and reducing the cost of care delivery.
Products:
Remote Patient Monitoring (RPM)
iUGO Care helps patients to monitor their health status anywhere, anytime using medical devices. With RPM, automated alerts are delivered to the care team if the patient’s condition starts to deteriorate to enable early, proactive interventions.
Chronic Care Management (CCM)
iUGO Care allows clinicians to track the time spent on non-face-to-face care and care coordination services provided to at-risk patients with multiple comorbidities. Other CCM services include medication tracking, service provider scheduling, and records management.
Principal Care Management (PCM)
iUGO Care can support patients with a single, but serious, chronic condition. With PCM, providers, typically a specialist, develop a disease-specific care plan, adjust medication regimens, and carry out complex decision-making.
Behavioral Health Integration (BHI)
iUGO Care supports patients with any mental, behavioral, or psychiatric health diagnoses by integrating mental health, psychiatric care, counseling, and addiction services with primary care. BHI supports primary care providers in delivering care with a focus on improving mental health outcomes.
Telemedicine
iUGO Care supports secure, HIPAA-compliant virtual visits between patients and their clinical care team providers. The Telemedicine platform gives patients and providers the flexibility to meet remotely using video conferencing, secure text messaging, or a phone call.
Transitional Care Management (TCM)
iUGO Care supports patients immediately after discharge from a hospital or other facility, as they transition to a community setting. TCM helps providers take responsibility for the patient’s care, including care coordination, interactive communication, and medical decision-making.
Remote Therapeutic Monitoring (RTM)
iUGO Care provides patients with ongoing care management focused on medication adherence and therapy response. iUGO Care’s RTM module focuses on the management of patients using medical devices that collect non-physiological data and allows clinicians and care providers to make treatment decisions based on real-time data.
Wound Care
iUGO Care includes functionality for remote wound monitoring between home visits or clinic appointments. Patients and clinicians can upload images of wounds to track the wound care process; measuring the size of the wounds, monitoring outcomes, and tracking dressing changes.
